{"id":210453,"date":"2024-04-11T08:13:40","date_gmt":"2024-04-11T12:13:40","guid":{"rendered":"https:\/\/today.uconn.edu\/?p=210453"},"modified":"2024-04-11T08:27:50","modified_gmt":"2024-04-11T12:27:50","slug":"advancing-approaches-to-physical-therapy","status":"publish","type":"post","link":"https:\/\/today.uconn.edu\/2024\/04\/advancing-approaches-to-physical-therapy\/","title":{"rendered":"Advancing Approaches to Physical Therapy"},"content":{"rendered":"<p>Daniele Piscitelli, assistant professor of <a href=\"https:\/\/kins.uconn.edu\/\">kinesiology<\/a> in the <a href=\"https:\/\/cahnr.uconn.edu\">College of Agriculture, Health and Natural Resources<\/a>, seeks to advance physical therapy practice through approaches like utilizing novel algorithms and incorporating the latest developments in neuroscience into rehabilitation.<\/p>\n<p>Despite the development of effective vaccinations, nearly four years after the emergence of COVID-19 in the U.S., people are still contracting severe cases of COVID-19. Critical illness due to COVID-19 can include symptoms such as pneumonia and acute respiratory distress syndrome.<\/p>\n<p><img decoding=\"async\" class=\"alignleft wp-image-209097 img-responsive lazyload\" data-src=\"https:\/\/today.uconn.edu\/wp-content\/uploads\/2024\/01\/cahnr_10th_blue-green-300x139.png\" alt=\"CAHNR 10th Anniversary of Health badge\" width=\"180\" height=\"84\" src=\"data:image\/svg+xml;base64,PHN2ZyB3aWR0aD0iMSIgaGVpZ2h0PSIxIiB4bWxucz0iaHR0cDovL3d3dy53My5vcmcvMjAwMC9zdmciPjwvc3ZnPg==\" style=\"--smush-placeholder-width: 180px; --smush-placeholder-aspect-ratio: 180\/84;\" \/>Piscitelli, a faculty member in the Doctor of Physical Therapy Program developed an algorithm to help clinicians determine if early mobilization is appropriate for patients with severe COVID-19 cases. This research was published in <a href=\"https:\/\/doi.org\/10.1080\/09638288.2023.2263370\">Disability and Rehabilitation<\/a>.<\/p>\n<p>Early mobilization is a clinical approach in which a physical therapist helps a critically ill patient resume basic movement like sitting up, stretching, and even walking in some cases.<\/p>\n<p>The paper combined a scientific review of existing research on this topic and clinicians\u2019 experiences with early mobilization in these patients.<\/p>\n<p>Current guidelines for early mobilization restrict the practice to patients within a certain threshold of oxygen support. However, recent evidence suggests there are benefits for getting patients outside of this threshold moving again. Further, no one had yet addressed what to do with patients who have multiple safety concerns, such as those with severe COVID-19.<\/p>\n<p>The team developed an algorithm to help clinicians in the Intensive Care Unit (ICU) decide whether to implement early mobilization for individual COVID-19 patients. The algorithm uses information about the patient\u2019s cardiovascular, pulmonary, skeletal, and neurological health. Then, using evidence-based criteria, if the patient meets certain standards, the clinician knows it is safe to engage the patient in early mobilization.<\/p>\n<p>\u201cThe algorithm takes into account all the systems,\u201d Piscitelli says. \u201cFor instance, by examining the cardiovascular, one can assess the status of blood pressure. Then, you look at the musculoskeletal system and you note, for example, that there are no unstable fractures.\u201d<\/p>\n<p>Critically, the patient and their family must consent to this treatment. Prior research has shown patients have better outcomes when they are part of the decision-making process in a patient-centered model of care.<\/p>\n<p>\u201cThis cannot be applied without the patient, family members, and all other members of the team,\u201d Piscitelli says.<\/p>\n<p>The algorithm has not been fully evaluated, but it is an important first step. It also has potential applications beyond COVID-19.<\/p>\n<p>\u201cWe think this kind of algorithm that we implemented can also be applied to other populations with respiratory distress,\u201d Piscitelli says.<\/p>\n<p>The next step will be to test this algorithm in ICUs throughout the country to determine if it produces better patient outcomes.<\/p>\n<p>Piscitelli also recently co-authored a book on translating neuroscience into physical rehabilitation practices, addressing a lack of such connections.<\/p>\n<p>Three of the authors of \u201c<a href=\"https:\/\/shop.elsevier.com\/books\/progress-in-motor-control\/levin\/978-0-443-23987-8\">Progress in Motor Control: From Neuroscience to Patient Outcomes<\/a>\u201d are licensed physical therapists (Piscitelli, Mindy F.
