UConn Health\u2019s outpatient anticoagulation clinic<\/a> is recognized as an Anticoagulation Center of Excellence for a second time.<\/p>\n

The three-year designation comes from the Anticoagulation Forum, a multidisciplinary nonprofit with a mission to improve the quality of care for patients taking antithrombotic medications, which are blood-thinning drugs for preventing or reducing the risk of clotting.<\/p>\n

\"Anticoagulation<\/a>The clinic is located at 11 South Road, just off UConn Health\u2019s main campus, in Farmington. Specially trained pharmacists and pharmacy technicians provide ongoing management of patients who are prescribed anticoagulants, which includes dosing, monitoring, education, and prescription needs.<\/p>\n

\u201cThe anticoagulation clinic functions to improve safety and efficacy outcomes by preventing new or recurrent thromboembolic events and avoiding adverse drug events for patients prescribed anticoagulants in a cost-effective manner, while allowing providers more availability of clinic time for direct patient care,\u201d says pharmacy clinical coordinator Anuja Rizal.<\/p>\n

Beyond their clinical responsibilities, they also mentor and train advanced pharmacy practice experience students and pharmacy residents, and are involved in the UConn School of Pharmacy\u2019s Anticoagulation Traineeship Certificate Program<\/a>.<\/p>\n

\u201cBeing recognized as an Anticoagulation Center of Excellence reflects the dedication and expertise of our team in delivering safe, consistent, and personalized anticoagulation management,\u201d says anticoagulant pharmacist Elizabeth Biron. \u201cMost importantly, it gives our patients confidence that they are receiving trusted care from a program focused on safety, outcomes, and continuous improvement.\u201d<\/p>\n

UConn Heath\u2019s anticoagulation clinic first earned the Center of Excellence designation in 2023.<\/p>\n

\u201cThe Centers of Excellence assessment provides a rigorous set of standards which are challenging for any service to attain,\u201d says Associate Vice President and Chief Pharmacy Officer Kevin Chamberlin. \u201cOver the life of the program, about 175 practices have achieved this recognition \u2014 only about one in four who take the assessment, pass.\u201d<\/p>\n
