Leapfrog is a national organization working to raise consumer awareness about the importance of choosing a safe and high-quality clinical care hospital. Twice a year it assigns its Leapfrog Hospital Safety Grades of either an A, B, C, D or F letter grade to hospitals across the country using 30 measures of publicly available hospital safety data. These indicators help assess how well hospitals are protecting inpatients from preventable errors, injuries and infections.
Each hospital’s data is collected annually from the Leapfrog Hospital Survey, the Agency for Healthcare Research and Quality (AHRQ), the Centers for Disease Control and Prevention (CDC), the Centers for Medicare and Medicaid Services (CMS) and the American Hospital Association’s Annual Survey and Health Information Technology Supplement.
