UConn Health Anticoagulation Clinic Again ‘Center of Excellence’

Recognizing care for patients taking antithrombotic medications

UConn Health’s outpatient anticoagulation clinic is recognized as an Anticoagulation Center of Excellence for a second time.

The three-year designation comes from the Anticoagulation Forum, a multidisciplinary nonprofit with a mission to improve the quality of care for patients taking antithrombotic medications, which are blood-thinning drugs for preventing or reducing the risk of clotting.

Anticoagulation Center of Excellence 2026-2029 graphicThe clinic is located at 11 South Road, just off UConn Health’s main campus, in Farmington. Specially trained pharmacists and pharmacy technicians provide ongoing management of patients who are prescribed anticoagulants, which includes dosing, monitoring, education, and prescription needs.

“The anticoagulation clinic functions to improve safety and efficacy outcomes by preventing new or recurrent thromboembolic events and avoiding adverse drug events for patients prescribed anticoagulants in a cost-effective manner, while allowing providers more availability of clinic time for direct patient care,” says pharmacy clinical coordinator Anuja Rizal.

Beyond their clinical responsibilities, they also mentor and train advanced pharmacy practice experience students and pharmacy residents, and are involved in the UConn School of Pharmacy’s Anticoagulation Traineeship Certificate Program.

“Being recognized as an Anticoagulation Center of Excellence reflects the dedication and expertise of our team in delivering safe, consistent, and personalized anticoagulation management,” says anticoagulant pharmacist Elizabeth Biron. “Most importantly, it gives our patients confidence that they are receiving trusted care from a program focused on safety, outcomes, and continuous improvement.”

UConn Heath’s anticoagulation clinic first earned the Center of Excellence designation in 2023.

“The Centers of Excellence assessment provides a rigorous set of standards which are challenging for any service to attain,” says Associate Vice President and Chief Pharmacy Officer Kevin Chamberlin. “Over the life of the program, about 175 practices have achieved this recognition — only about one in four who take the assessment, pass.”
